PLEASE READ THIS SECTION FIRST. This is a TEMPORARY interface for submitting team coach and staff coach recommendations of players for the US Youth Soccer ODP and the YOP programs. The contents of the forms on the following pages will be transmitted by e-mail to the Associate Technical Director. These pages have been upgraded to allow submission by most all browsers, including AOL. An acknowledgement page is now generated after successful submission.

There are alternate hard-copy pdf forms available from the following pages which you may use if your browser has any difficulties or if you would prefer to submit a hard copy. The "fillable" pdf can be completed with your Adobe Acrobat Reader, printed, and mailed or faxed. If you have the complete Adobe Acrobat (not just the free Reader), you can save the "filled" form and e-mail it. The "plain" pdf is for printing and filling out by hand.
